Informs senior citizens about available resources, makes appropriate referrals and offers outreach services to help each person to achieve realistic goals.
We have available information about living wills, medicine assistance programs, insurance counseling, long term care, nursing facilities, hospitals, and services available through other local agencies.
We make referrals to other local agencies when we find a client is eligible for a service. This is only done with the client's permission. Examples of referrals are to agencies like Department of Social Services for: Medicaid Transportation, Food Stamps, SSI; Community Long Term Care for: Meals, Home Care Aides; Home Health for: In-Home Health Care Aides, Ramps, Physical Therapy. For more information on other county resources, contact the Chesterfield County Council on Aging.
We go out in the county to speak at churches, senior groups, and door-to-door, making the county aware of the services available through Council on Aging. If you or a group would like for us to visit you please contact us.
